This is a Contract between Tetyana Mokrytska and the pet owner whose signature appears below (hereinafter called "Owner").

  1. Owner agrees to pay the rate for boarding in effect on the date pet is checked into my house.
  2. Owner further agrees to pay all costs and charges for special services requested, and all veterinary costs for the pet during the time said pet is in the care of Tetyana Mokrytska.
  3. Owner further agrees that the pet shall not leave boarding house until all charges are paid to Tetyana Mokrytska by Owner.
  4. By signing this Contract and leaving his/her pet with Tetyana Mokrytska, Owner certifies to the accuracy of all information given about said pet .The Owner further agrees to be solely responsible for any and all acts or behaviour of said pet while it is in the care of Tetyana Mokrytska.
  5. Owner specifically represents that he or she is the sole owner of the pet, free and clear of all liens and encumbrances.
  6. Owner specifically represents to Tetyana Mokrytska that the pet has not been exposed to rabies, distemper, parvo virus, feline leukemia or other contagious diseases within a thirty day period prior to boarding.
  7. In case owner does not pick up the pet within 10 calendar days after the day the pet was due to be picked up, the pet shall be deemed to be abandoned. Tetyana Mokrytska will place pet at a shelter or home of Tetyana Mokytska s choice. If a shelter or a home is not available, Tetyana Mokrytska reserves the right to do whatever is necessary.
  8. If the pet becomes ill or if the state of the pet's health otherwise requires professional attention, Tetyana Mokrytska, in her sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the pet, and the expenses thereof shall be paid by Owner.
  9. Owner certifies that all medical conditions have been revealed to Tetyana Mokrytska.
  10. Daily boarding charges begin on the date entered on this contract. Customer agrees to notify this boarding house in advance if there is any change in the date the boarding animal is to be picked up. No boarding animal will be released until all charges are paid in full.
  11. Dogs must be up to date on the following vaccinations: Distemper, Parvo, Lepto, Hepatitis, Corona, Bordetella, Kennel Cough and Rabies.
  12. Pets with fleas/ticks will be bathed/dipped at owner's expense in order to maintain flea control in the boarding house.
  13. I agree to provide a safe and sanitary boarding environment for your pet. Only pets certified in this agreement by their owners as current in all vaccinations will be admitted.
  14. Be aware that pets, especially older pets, are placed under a great deal of stress because of the removal from their normal home environment. This stress can cause latent (dormant) physical conditions such as heart, liver and kidney disorders to become active or compromise the immune system resulting in illness such as diarrhea, kennel cough, depression, nervousness, dehydration, and even death.
  15. Tetyana Mokrytska reserves the right to refuse service for any reason.
  16. Tetyana Mokrytska does not assume and shall not be held responsible for any liability with respect to the pet listed in this agreement of any kind, character, or nature whatsoever, arising out of or from the boarding of this pet, or any damages, which may occur from any cause whatsoever including loss by fire, theft, running away, death , injury to persons, pets, or property, or death or injury to any other pet caused by the within named pet whether this pet be on the premises of Tetyana Mokrytska or not, and the owner of pet agrees hereby to be and is solely responsible for any and all acts of behaviour of said pet at any time within the term and time of this agreement.

If neither of the veterinary offices named above is available, I authorize Tetyana Mokrytska to take my pet to another veterinary office for treatment. I understand that Tetyana Mokrytska cannot be held responsible for the results of the veterinary treatment or the loss of my pet.
This agreement is valid starting on the date below whenever Tetyana Mokrytska cares for my pet:
